See 10 – 12 patients daily with strong APP support
Minimal call with nurse triage support system
Value-based care experience preferred
Board-certified or board-eligible in internal medicine with geriatric focus required
Experience with HEDIS measures and HCC coding valued
Spanish language skills appreciated but not required
